This page contains forms you may need for everyday operations in a GNETS classroom.


Parent Communication:



This is a sample description of a class level system. Please send home a similar description to all parents at the beginning of the school year. I frequently hear from parents that they do not understand the level system and I think this will go a long way to help clarify any question.

Monthly paperwork:



This is the Master student attendance page for 2013. MH services at school include counselors from outside agencies visiting the child at school for counseling sessions. MH services outside of school include counseling, psychologist, psychiatrist or visiting a doctor for management of psychiatric medications. Please remember to turn this in every month via email to rmallen48@nwgnets.org no later than the 5th.



Daily student attendance form. Please be in contact with your school attendance secretary so that our data regarding absences, withdrawal and entry dates are accurate. Reference your folder from the beginning of this year to mark FTE count properly at the top of each month. Email this to rmallen48@nwgnets.org by the 5th of each month.


If you have a student(s) going out for classes please put this letter in the teachers mailbox on a weekly or biweekly basis. In the past we have had several instances where a teacher is giving a student 100% everyday but was actually having problems from the student. This allows additional data to be collected on behavior. Also if you receive a completed letter from a teacher and it shows missing assignments please work with the out class teacher and student to get those completed as soon as possible.

RESA Mileage reimbursement form, current rate is 0.55 per mile. Be sure to save the completed form to your computer for your records. Then print it out, sign it and mail it to Pam Warren at RESA.


Homebound services are recorded on the Catoosa County form and turned in to Amanda. Remember you must provide 3 hours per week of instructional time to the student you serve. You may claim one hour of planning time for homebound a week.



Behavior Documentation:



This is a template point sheet for use in recording behavior data. Mastery for a positive day must be set at 90%. Times and totals may be adjusted for individual students. If an IEP goal is the same as one of the standard behaviors (1-3) simply make it bold so your para will recognize it as an IEP goal and will include it in the behavior graph data chart.


This template is for recording the students daily percentage on each of their IEP behavior goals. At the end of the month each column should be averaged. The average monthly scores should be used to complete IEP progress reports and should be graphed at the end of the year to show progress.


SWIS forms are used to document any behavior incident that results in a level drop or administrative involvement. Please remember you can only check one box per category. Please check the box to indicate the antecedent behavior, ie the first behavior that occurs. You can describe any escalation from that point in the narrative portion of the form. When completed these form are emailed to Amanda (aoxford@catoosa.k12.ga.us or aoxford@nwgnets.org for Walker). After I review and approve your form it should be sent home for the parent and student to sign. Keep a copy in case it is not returned, make a note of the date sent home and file in the student data notebook.


The first four pages of this document are the Northwest GA Education Program's restraint guidelines, the last page is the restraint report that should be used to document ANY restraint. Be sure to submit completed restraint forms to Amanda ASAP via fax or pony. Always call or text Amanda the day the restraint occurs.

Human Resources:


Online Leave Form Complete for leave requests.


For Workers comp. packet go to the human resources tab on www.nwgaresa.com. In the event of an employee injury, immediately contact Amanda. Complete the packet and fill out the yellow prescription card page from your classroom kit. Punch out the yellow card to take with you to the doctor and pharmacy. Look at your pink workers compensation panel to select an approved physician. Fax the entire packet to Pam Warren at RESA (706__)__ 389-9539 and give the originals to Amanda.



Paraprofessional major medical release. To be signed by doctor before returning from absence. (Surgery, major illness, hospitalization, injury, childbirth, etc)
